Completed Authorization to Release Medical Records
If you the patient are sending someone to pick up items on your behalf,
you must indicate that person's name on the HIPAA form where required
and add a phone number where you the patient can be contacted.
Cost
There is no charge for the first set of films or CD's.
If a second set is requested a nominal fee will be charged.
